1. Customer must have purchased one new XUS LED Lighting product for every cobra light to be replaced.
2. Complete the Information Form below and submit.
3. After XUS LED Lighting verifies the information and your prior purchase of our lighting products, we will email a Recycling Authorization to the address included in the form.
Please DO NOT SEND cobra fixtures without this Recycling Authorization.
4. Once authorized, ship replaced cobra fixtures to:

(shipping cost is responsibility of customer)
5. Credit will be up to USD $100 maximum for each cobra housing, depending on usability. This non-cash credit will apply to your next XUS LED Lighting purchase(s).
6. After authorization, you may also return XUS packaging for recycling and receive $10 credit toward your next purchase.
